Enacted by the Deficit Reduction Act of 2005, the Money Follows the Person (MFP) Rebalancing Demonstration is part of a comprehensive, coordinated strategy to assist States, in collaboration with stakeholders, to make widespread changes to their long-term care support systems. With the history and strength of the Real Choice Systems Change (RCSC) grants as a foundation, this initiative will assist States in their efforts to reduce their reliance on institutional care, while developing community-based long-term care opportunities, enabling the elderly and people with disabilities to fully participate in their communities. In 2007, CMS awarded $1,435,709,479 in MFP grants with States proposing to transition over 34,000 individuals out of institutional settings over the five-year demonstration period.30 States and the District of Columbia were awarded grants. One State has withdrawn their participation. Information on a summary of the awards is available under downloads below. CMS is offering an ongoing series of reports completed as part of the CMS MFP National evaluation. This first report assesses the scope of the program—profiling the Medicaid population in long-term institutional care eligible for MFP, their rates of transition to the community before MFP began, and the potential change in transition rates if States reach their program goals. The second report features "Implications of State Program Features for Attaining MFP Transition Goals." The report presents how State variation in level of experience and infrastructure to undertake large-scale transition programs can affect the pace and degree of MFP program implementation. The report also outlines how such differences could affect the likelihood of meeting transition goals. Please see the complete reports under "Downloads" below.
