Description The Community Based Care Transitions Program (CCTP) goals are; to reduce hospital readmissions, test sustainable funding streams for care transition services, maintain or improve quality of care, and document measureable savings to the Medicare program. The demonstration will be conducted under the authority of section 3026 of the Affordable Care Act of 2010.

Upcoming Panel Review Dates

The dates posted here represent published review dates. We have additional panels convening regularly based upon application volume. Applications will be received and reviewed on a rolling basis as funding permits. The CCTP webpage will read "solicitation closed" when we reach our funding ceiling and are no longer reviewing applications. We anticipate reviewing applications into the first half of 2012.

February 17, 2012 - Applications must be received by January 27th to be considered for this review.
March 8- Applications must be received by February 16th to be considered for this review.
March 27- Applications must be received by March 6th to be considered for this review.
April 10 - Applications must be received by March 20th to be considered for this review
April 26 - Applications must be received by April 5th to be considered for this review
May 10 - Applications must be received by April 19th to be considered for this review
May 30 - Applications must be received by May 9th to be considered for this review
June 11 - Applications must be received by May 21st to be considered for this review
June 28 - Applications must be received by June 7th to be considered for this review

We are pleased to announce the first sites selected for the Community Based Care Transition Program. Please see the links below for the list of sites and an updated fact sheet. As noted above, we continue to accept applications and look forward to selecting additional sites in the near future.
