Provider Service Plan Template In accordance with TDL 12012, the Centers for Medicare & Medicaid Services suspended the annual submission of the Provider Service Plan (PSP) for Legacy Title XVIII and Medicare Administrative Contractors (MACs) required by the Internet Only Manual (IOM) 100-09, Chapter 6, Section 20.7.1. Newly awarded MACs shall submit a PSP in accordance with the deliverable schedule. This format should be used effective January 1, 2012. If the PSP is due prior to January 1, 2012, you should use the current format. If you have any questions regarding the PSP, contact providerservices@cms.hhs.gov. Education Activity Report Template Medicare Contractors are required to prepare and submit a semi-annual Education Activity Report (EAR) in accordance with section 20.7.2 of IOM Pub-100-9, Chapter 6. The new EAR format should be used effective January 1, 2012. If the EAR is due prior to January 1, 2012, you should use the current format.
