Purpose

The Pharmacy Plus template provides a pre-designed application for states to use to submit section 1115 pharmacy demonstration programs. The template sets out choices for states within the parameters for the demonstration program, and pop-up instructions assist states with completing the application. To complete the Pharmacy Plus application, a state needs only to check the appropriate boxes, insert its financial information, and send the proposal to CMS. Using the Pharmacy Plus application, and following the guidelines, will increase the speed of federal review.
