Shared Savings Program ACO Agreement

Once an applicant has been approved to participate in the Medicare Shared Savings Program, the applicant will be required to sign a Medicare Shared Savings Program Accountable Care Organization Participation Agreement [PDF, 103KB] with CMS to comply with the regulations of the program under Title 42 Code of Federal Regulations Chapter IV, part 425 and to participate for a period of not less than three years.  In addition, ACOs must complete a Data Use Agreement [PDF, 1MB] .
