An updated version of the Technical User Guide for Hospice Quality Reporting Data Entry and Submission is available in the Downloads section at the bottom of this page. The changes to this guide since version 1.3 include:
- The addition of QTSO helpdesk information
- An updated screenshot for “Figure 5”
- The change to the user account activation email address
No changes to the steps for registration, data entry, and submission have been made.
This guide includes step by step instructions for user registration, entering data, attesting and submitting data to CMS. This is the primary reference source for this data entry and submission website.

The link to the Hospice Quality Reporting Program Data Entry and Submission website is now available: https://hospice.qtso.com. This link is also located below in the “Related Links” section at the bottom of this page. This link will take you to the Hospice Quality Reporting Data Entry and Submission home page where users may begin registration prior to the January 1st start date for Hospice Quality Reporting Program (QRP) data submission.
A very important note about Registration: There is only ONE user account allowed per hospice CCN (Medicare provider number).
Any registration of a hospice user account for a specific hospice CCN is ONLY for actual live or “production” data entry and submission. This means: do NOT register for a user account if you do NOT intend to complete data entry, attestation and submission tasks for a specific hospice provider. Also, do NOT register for casual viewing purposes as this will directly impact your hospice’s ability to register a user who will submit hospice QRP data.
Again, there is only ONE user account per CCN allowed for the submission of QRP data for both the structural QAPI measure and the NQF #0209 Pain measure. Therefore, it is very important that providers do their best to plan and think through assigning the task of data entry and submission PRIOR to registering for a user account. The person tasked with this responsibility of data entry and submission should be available throughout both submission periods to complete the submission for the Structural Measure and the Pain Measure data.
If you do NOT intend to complete the tasks of registration, data entry, attestation and submission for a specific hospice provider please do not register for an account.

Deactivation of user accounts
In circumstances where a new user MUST replace an existing user, CMS has established a User Deactivation process. This process allows the deactivation (or deletion) of an existing user account to enable a new user access to the hospice CCN for data entry and submission of QRP data.
In instances when this process is required it is vitally important that providers understand the following information:
- When a user account is deactivated, all data entered and/or submitted by the user on behalf of the provider is deleted.
- This means all data previously entered or submitted to CMS will be deleted and no longer associated with the provider. The hospice will not have access to that data, they will not be able to see what was previously entered once the account is deactivated.
- We strongly recommend that you print each measure page for your records.
- All data previously entered and/or submitted MUST be reentered/submitted by the new user
- This means if a user account is deactivated prior to the end of the January 31st submission deadline, the new user must re-enter and/or resubmit the structural measure data prior to January 31, 2013.
- This also means if a user is deactivated AFTER the end of the January 31st submission deadline, there is no ability to re-enter and/or resubmit that data.
- Consideration will be given to hospices who, due to circumstances well beyond their control, must request deactivation after completing structural measure data entry, attestation and submission by the 1/31/2013 deadline.
- For these reasons, CMS strongly encourages the hospice provider to plan as best they can that the person assigned this responsibility be available throughout both reporting periods.
If the deactivation process must be utilized, a completed User Deactivation form along with provider letterhead must be submitted to the QTSO help desk via email help@qtso.com or fax 888-477-7871.
The Hospice Individual User Deactivation Request form is available by contacting the QTSO Help Desk at 1-877-201-4721. The Hospice Quality Reporting Program Data Entry and Submission WebEx recording and training slides are now available on the Hospice Training page of the QTSO website, https://www.qtso.com.
No registration is required to view the WebEx; Hospice providers can view the recorded WebEx at their convenience and as many times as needed until April 2013. Hospices can access the WebEx via: https://www.qtso.com/hospicetrain.html.
The Hospice Quality Reporting Program Data Entry and Submission WebEx demonstrates how to:

For the Hospice Quality Reporting Program requirements affecting the FY 2014 payment determination, hospices will submit two measures: The Structural/QAPI measure and the NQF #0209 measure. The data collection period for both measures is Oct 1-Dec 31, 2012. The structural measure must be submitted by January 31, 2013 and the NQF #0209 by April 1, 2013.

Vendor/Third Party Data Submission
CMS permits vendors/third parties to submit quality data on behalf of providers. All agreements and arrangements are performed entirely between the provider and the vendor/third party and there are no CMS-required procedures related to third party submissions.
CMS does hold the provider fully accountable for the truthfulness, completeness and accuracy of their data, regardless of who the provider assigns to submit such data. Providers independently elect who is designated to submit their quality data to CMS, and third party submitters acting on behalf of providers are required to acknowledge they are duly authorized to submit the quality data at the time of submission. All data submission requirements, including deadlines for data submission, are applicable to third party submissions.
Please note, the HQRP web-based data submission tool is set up to receive one hospice provider’s data per submission, therefore “scripting” or batch submissions are not allowed.

User Guides
User Guides for the Hospice Quality Reporting Program (data collection October 1 – December 31, 2012 and data submission January 1 - April 1, 2013) are now available on the CMS Hospice Quality Reporting website.
There will be two User Guides available for provider use. We encourage providers to prepare for data collection and submission by downloading and reading the available Guides now:
1. User Guide for Hospice Quality Reporting Data Collection -- is available in the Downloads section of the “Spotlight” portion of this webpage. This User Guide will instruct hospice providers on data collection processes for the Hospice Quality Reporting Program (HQRP). Data Collection activities for the first year of the HQRP will take place October 1, 2012 – December 31, 2012.
2. Technical User Guide for Hospice Quality Reporting Data Entry and Submission -- is available in the Downloads section of this webpage. This User Guide will instruct hospice providers on data submission processes and provide details about the data entry website for the HQRP. Data Submission activities for the first year of the HQRP will take place January 1 – April 1, 2013.
• Please note: the data entry site is not yet available for provider use; the site is scheduled to go live January 1, 2013.

Hospice Quality Reporting Data Submission Update
CY2013 will be the first year in which Hospices will be required to submit quality data. The data entry form previously described here will be updated to accommodate the addition of the pain measure (NQF #0209). This first year of mandatory reporting will require submission of the QAPI measure data in January and the pain measure data in April. The User Guide will also be updated to include instruction for the pain measure.
Further updates related to the data submission details for the first mandatory reporting period will be added to this site as they are finalized.
