November 29, 2012

The MDS 3.0 RAI Manual (v1.09), recently published on November 7, has minor inconsistencies in the Change Table files and several RAI Manual pages.

The two pdf files below contain corrected pages and updates for the previously published MDS 3.0 RAI Manual (v1.09) Change Tables, and additional corrected replacement pages for the MDS 3.0 RAI Manual (v1.09).

Providers who downloaded and printed the "All Change Tables and Pages for Version 1.09" ZIP file or the MDS 3.0 RAI Manual (v1.09), will need to download and print both supplementary documents.

MDS 3.0 QM User’s Manual V6.0 - Contains detailed specifications for MDS 3.0 quality measures.

Quality Measure Identification Number by CMS Reporting Module table - documents CMS quality measures calculated using MDS 3.0 data and reported in a CMS reporting module.  A unique CMS identification number is specified for each QM.
