**Updated April 16, 2014- Special Open Door Forum (ODF): Suggested Electronic Clinical Template for Home Health for Tuesday, April 22, 2014 3:00pm – 4:00pm Eastern Time. This call will be conference call only. The Centers for Medicare & Medicaid Service will host a series of Special Open Door Forum (SODF) calls to allow physicians and other interested parties to give feedback on data elements for the Suggested Electronic Clinical Template for Home Health.  In order to enhance physician understanding of medical documentation requirements to support orders for Home Health services, CMS has developed a list of clinical elements within a Suggested Electronic Clinical Template that would assist physicians when documenting the Home Health (HH) face-to-face encounter for Medicare purposes. While not intended to be a data entry form, the template will describe the clinical elements that CMS believes would be useful in supporting the documentation requirements for coverage of Home Health services. CMS will work in collaboration with the DHHS Office of the National Coordinator for Health IT (ONC) and the electronic Determination of Coverage (eDoC) workgroup which are focused on giving practitioners access to payer approved tools for the electronic submission of medical documentation. You can find the proposed document by going to:
http://www.cms.gov/Research-Statistics-Data-and-Systems/Computer-Data-and-Systems/Electronic-Clinical-Templates/HomeHealthHHElectronicClinicalTemplate.html. Comments on the document can be sent to eclinicaltemplate@cms.hhs.gov.  If you wish to participate, dial: 1-800-837-1935 & Conference ID: 20361722.  We look forward to your participation. **Updated April 4, 2014- Special Open Door Forum (ODF): Part D and Hospice for Tuesday, April 8, 2014 1:00pm – 2:00 pm Eastern Time. This call will be conference call only. This Special ODF will provide an opportunity to allow hospices and other interested parties to ask questions of CMS subject matter experts on the “Part D Payment for Drugs for Beneficiaries Enrolled in Hospice—Final 2014 Guidance” issued on March 10, 2014.  You can find the slides for this ODF on the “Spotlight” section of the Hospice Center Webpage by going to: http://www.cms.gov/Center/Provider-Type/Hospice-Center.html.   On December 6, 2013, CMS issued a memorandum seeking to clarify the criteria for determining payment responsibility under the Part A hospice benefit and Part D for drugs for hospice beneficiaries. Based on these clarifications we suggested revised expectations for Part D sponsors to prevent duplicate payments for drugs covered under the hospice benefit or waived through the beneficiary’s hospice election. We issued that guidance for industry review and comment.  You can find the Final 2014 Guidance by going to:  http://www.cms.gov/Medicare/Medicare-Fee-for-Service-Payment/Hospice/Downloads/Part-D-Payment-Hospice-Final-2014-Guidance.pdf .  Questions on the Final 2014 Guidance can be sent to PARTDPOLICY@cms.hhs.gov. When submitting questions to this mailbox related to this guidance, please include “Hospice” in the subject line. If you wish to participate, dial: 1-(800) 837-1935 & Conference ID: 25598571. We look forward to your participation. Please see the Downloads section below for the full call announcement. Thank you for your continued interest in the CMS Open Door Forums.
