In April CMS released a new version of the 588 form, the form that is used to sign up for direct deposit of Medicare payments or EFT (electronic funds transfer). Unfortunately the form lacks a date box and it only has spaces for a 12 digit account number. |

July 19: Both problems have been fixed. The updated form now has space for a 13 digit account number and a date box has been added.The updated form can be found at: http://www.cms.gov/cmsforms/downloads/CMS588.pdf
