NOTE - CMS will not accept requests which have been uploaded and encrypted to a third-party vendor site.  If you wish to protect the documents from unauthorized disclosure, you probably already have the capability to password protect or encrypt your documents using your operating software compression utility (e.g. WINzip) or your desktop software (e.g. MS-Office).  Refer to your organization's information technology (IT) support services for assistance.

Effective June 1, 2012, all DUA request submissions must follow the e-mail Subject line instructions located on the DUA-Forms page (use the link in the left hand menu).

For all new Data Use Agreement (DUA) requests, use the links in the left margin for specific instructions on making requests for CMS data.

Free assistance is available for:

• Federal Agencies (excluding CMS contractors/grantees)
• Federal Grantees
• Medicare Current Beneficiary Survey (MCBS) data requestors
• Researchers (RIFs) (excluding Providers, LDS and DSH requesters)
