Step 2: Meaningful Use Core Measures

If you are a Medicare Eligible Professional participating in the EHR Incentive Program, you will need to successfully demonstrate meaningful use of certified EHR technology. This application can serve as a guide of whether or not you meet the program’s Meaningful Use requirements, but is not an actual attestation and does not guarantee that you will meet the program’s qualifications.

For the Medicare EHR Incentive Program, the reporting period in the first calendar year of participation is 90 days. The reporting period for all subsequent years will be the entire calendar year. Providers should base their attestation information on the appropriate reporting period when using this tool.

For the Medicaid EHR Incentive Program, EPs do not need to successfully demonstrate meaningful use in their first year of participation in order to receive an incentive payment. The reporting period in the second calendar year of participation is 90 days. The reporting period for all remaining calendar years will be the entire calendar year. Providers should base their attestation information on the appropriate reporting period when using this tool.

Instructions: Users must adequately answer each measure they intend to meet by either correctly filling in the numerator and denominator values, or successfully marking down an exclusion (for those that apply). For measures that provide more than one exclusion option, users should only answer “yes” for the one exclusion they intend to claim and leave the other blank. Note, users must mark off each of the five meaningful use menu measures they intend to meet by selecting the blank checkbox located above each menu measure. Similarly to the actual attestation system, users are not able to submit for more than five menu measures.





Medicare and Medicaid EHR Incentive Program
