Enroll as a DMEPOS Supplier

Suppliers who receive Medicare reimbursement for durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS) are required to:

  • Obtain DMEPOS accreditation from a CMS-approved organization
  • Enroll in the Medicare program as a DMEPOS supplier
  • Post a surety bond to their enrollment contractor

If your business doesn’t dispense or furnish DMEPOS, you should use the Medicare Enrollment Guide for Providers & Suppliers.

If you’re enrolling a hospital, critical care facility, skilled nursing facility, home health agency, hospice, or other similar institution, you should refer to the Medicare Enrollment Guide for Institutional Providers.

Medicare Enrollment Guide for DMEPOS Suppliers

Important Information Regarding Changes to the DMEPOS Enrollment Contractor: Effective Nov. 7, 2022, the National Supplier Clearinghouse (NSC) no longer processes Medicare enrollment applications for DMEPOS suppliers. The National Provider Enrollment DMEPOS East and West contractors now process all DMEPOS enrollment applications. Find your enrollment contractor (PDF).

Step 1: Obtain DMEPOS Accreditationi

A CMS-approved accreditation organization will verify that your business meets the required DMEPOS Quality Standards (PDF) and conduct periodic, unannounced site visits. Find a CMS-approved accreditation organization (PDF).

Step 2: Get an NPI for Each Practice Locationii

If you already have an NPI for each practice location, skip this step and proceed to Step 3.

NPIs are issued through the National Plan & Provider Enumeration System (NPPES). You can apply for an NPI on the NPPES website. Not sure if you have an NPI for each practice location? Search the NPI Registry.

Step 3: Complete the Enrollment Application and Electronic Funds Transfer Authorization Agreement CMS-588

Enroll using PECOS,iii the online Medicare enrollment system. PECOS has video and print tutorials and will walk you through your enrollment to ensure your information is accurate. Complete the online PECOS application.

Step 4: Pay the Medicare Application Fee

The Medicare Application Fee for 2023 is $688. You can pay the fee on the PECOS Medicare Fee Payment page.

Step 5: Work With Your Enrollment Contractor

You can check in with your enrollment contractor regarding the status of your application. Find your enrollment contractor (PDF).

Step 6: Post a Surety Bond to Your Enrollment Contractor

DMEPOS suppliers are required to post a surety bond in the amount of $50,000 for each NPI they maintain. The U.S. Department of the Treasury maintains a list of certified surety bond companies.

Keep Your Information Current

It’s important to keep your enrollment information up to date. To avoid having your Medicare billing privileges revoked, be sure to report any change within 30 days. Changes include, but are not limited to:

  • a change in ownership
  • an adverse legal action
  • a change in practice location

If you applied online, you can keep your information up to date in PECOS. If you applied using the paper application, you will need to submit your form to update information.
