American Indian Health and Family Services is a non-profit health organization serving a large number of the uninsured or under insured and also served as a 2013 Navigator grantee. Funds will be used to reach its target audience of American Indians, Alaskan Natives, Latinos, and Asians through the mass mailing of brochures, flyers and informational literature geared toward specific populations, hosting and attending educational forums, increasing social media networking and advertising efforts, publications, and training expert personnel to assist consumers in enrollment and renewals.

Forest County Potawatomi CommunityMI

Forest County Potawatomi Community will coordinate with 16 tribes, with each tribe having its own Navigator outreach office. Navigators will be trained and housed within each tribal community.

Montana Wyoming Tribal Leaders CouncilMT

The Tribal Leaders Council (TLC) includes member Tribes from 7 reservations in Montana and they plan to provide assistance to American Indians residing on and near the Reservations, and in particular, major urban areas. They will utilize existing outreach projects in the communities by coordinating with tribal health departments and by participating in community health events.

Ponca Tribe of NebraskaNE

The Ponca Tribe of Nebraska, also a 2013 Navigator grantee, will use funds to continue providing outreach, education, and enrollment services to American Indians residing in the fifteen counties that constitute the Ponca Service Delivery Area. Funds will be used to hire Ponca Navigators, and assistance will be delivered through their five office locations, in person, and via videoconferencing.

The Great Plains Tribal Chairmen’s Health Board (GPTCHB) is a community based consumer focused non-profit that served as a 2013 Navigator grantee. It plans to continue providing enrollment assistance to American Indians residing on and near the eight Reservations in South Dakota, the four Reservations and one Indian Service Area in North Dakota, and those residing in major urban areas served by Urban Indian Health Centers in these two States.

The Great Plains Tribal Chairmen’s Health Board (GPTCHB) is a community based consumer focused non-profit that served as a 2013 Navigator grantee. Urban Indian Center of Salt LakeUT

Urban Indian Center of Salt Lake will target tribal and urban American Indian (AI) members of the Ute Tribe of the Ouray and Uintah Reservation, the Confederated Tribe of the Goshutes of Ibapah, and the Urban Indian Center of Salt Lake (UICSL) who reside along the Utah’s Wasatch Front. It plans to conduct outreach at the Indian Training & Education Center, and by making regular visits to the Native American Trading Post, homeless shelters, domestic violence shelters, and halfway homes.

The North Dakota Center for Persons with Disabilities will use Navigator grant funds to establish a coordinated network of regional assisters. Oklahoma Community Health Centers, Inc.OK

Oklahoma Community Health Centers, Inc., a 2013 Navigator grantee, is a consortium comprised of a variety of non-profit organizations that will apply a statewide approach to providing enrollment assistance to Oklahomans. Consortium members will have Navigators located in 57 cities across the state, enabling the consortium to provide services in 45 counties in Oklahoma. Among other activities, funds will be used to target outreach to rural, uninsured populations, including Native Americans, individuals with limited English proficiency, and increased outreach to Burmese and Vietnamese communities.
