Medicare EDI Enrollment

You must complete a Medicare EDI Enrollment Form and the EDI registration form before carrying out EDI transactions either directly with Medicare or through a billing service or clearinghouse. By completing these forms, you agree to accept responsibility for safeguarding patient data and billing correctly.

EDI Enrollment Forms are specific to each MAC. For more information about the form, find your MAC’s website. An organization with multiple components that has assigned NPIs and Provider Transaction Access Numbers (PTANs) may choose to execute a single EDI Enrollment Form on behalf of the organization.
