Enter the billing name, address, ZIP Code, and telephone number of the health care professional or supplier providing the service. For the 837P, submit the billing name in the 2010AA NM1. Submit the billing address and ZIP Code in the 2010AA N3 and N4. Submit the billing telephone number in the 2010AA PER. Medicare requires a 9-digit ZIP Code in this field. The billing health care professional or supplier’s address can’t be a post office (PO) box.
You must report certain events involving your enrollment record no later than 30 days after they occur. For example, a MAC can return the claim if the address in item 33 doesn’t match your enrollment record.
