Billing Codes

The CMS-1500 and 837P require CPT and HCPCS codes.

CPT

Use HCPCS Level I and II codes to code procedures on all claims. Level I CPT codes describe medical procedures and professional services. CPT is a numeric coding system maintained by the AMA. The CPT code book is available from the AMA.

HCPCS

Level II of the HCPCS is a standardized, alpha-numerical coding system that’s used primarily to name products, supplies, and services not included in Level I CPT codes, like ambulance services and DMEPOS when used outside a physician’s office or injections administered within a physician’s office or clinic.
