National Provider Identifier

All health care providers who are HIPAA-covered entities must get an NPI. HIPAA-covered entities like health plans, health care clearinghouses, and certain health care providers use the NPI to identify health care providers in HIPAA standard transactions.

You can apply for an NPI from NPPES in 1 of 3 ways:

  1. Apply through a web-based application process. Visit the NPPES webpage.
  2. Complete, sign, and mail a paper application form (CMS-10114) to the NPI Enumerator. You can ask for a hardcopy application through the NPI Enumerator by calling 1-800-465-3203 or TTY 1-800-692-2326. Email requests to customerservice@npienumerator.com.
  3. Give permission to have an Electronic File Interchange Organization (EFIO) send the application data on your behalf through a bulk enumeration process. For more information on this choice, visit the How to Apply webpage.
