National Provider Identifier
All HIPAA-covered health care providers need an NPI.
The NPI identifies you in HIPAA standard transactions with health plans, health care clearinghouses, and health care providers.
You can apply for an NPI from NPPES in 1 of 3 ways:
- Visit the NPPES webpage to use a web-based application process.
- Complete, sign, and mail a paper application form (CMS-10114) to the NPI Enumerator. Get a hardcopy application through the NPI Enumerator by calling 1-800-465-3203 or TTY 1-800-692-2326, emailing requests to customerservice@npienumerator.com, or writing to NPI Enumerator, 7125 Ambassador Road, Suite 100, Windsor Mill, MD 21244-2751.
- Allow an Electronic File Interchange Organization (EFIO) to send the application data on your behalf through a bulk enumeration process. For more information, visit the How to Apply webpage.
