Claim Coding Information
Providers use a standardized coding system called Level II of the HCPCS mainly to identify products, supplies, and services not included in the CPT-4 codes, like ambulance services and DMEPOS when used outside a physician’s office. Because Medicare and other insurers cover a variety of services, supplies, and equipment that CPT-4 codes don’t identify, the American Medical Association (AMA) and CMS established Level II HCPCS codes for submitting claims for these items.
