Report line item dates of service for the services listed below on all bills containing revenue codes, procedure codes, or drug codes.	Patient or provider records

Community Mental Health Center (CMHC) and hospital claims require service dates. This includes claims where the From and Through dates are equal. Facilities exempt from this requirement are:

Critical Access Hospitals (CAHs)
Indian Health Service (IHS) Hospitals
Hospitals in American Samoa, Guam, and the Northern Mariana Islands

Enter a single Line Item Date of Service (LIDOS) for each iteration of every revenue code on all outpatient bills and on inpatient Part B bills. Services rendered 5 times during a billing period must have the revenue code and HCPCS code entered 5 times, once for each service date.
