Decide If You Want to Participate (Continued)

You must complete the Medicare Participating Physician or Supplier Agreement (CMS-460) form to participate in the Medicare Program as a provider or supplier.

You have 90 days from when you enrolled to decide if you want to be a participating provider or supplier. The only other time you can change your participation status is during the open enrollment period, generally from mid-November through December 31.
