Data Specifications Version 1.20 Correction 1
The required corrections for 5 MDS Items in Version 1.20 all involve records combining a quarterly assessment with a Medicare PPS assessment (REC_TYPE QM). The required changes are as follows:
- Change the status for item G6f (Modes of Transfer: None of Above) on REC_TYPE QM from "Active" to "State Option".
- Change the status for item H2e (Bowel Elimination Pattern: None of Above) on REC_TYPE QM from "Active" to "State Option".
- Change the status for item H3j (Continence Appliances and Programs: None of Above) on REC_TYPE QM from "Active" to "State Option".
- Change the status for item J1p (Problem Conditions: None of Above) on REC_TYPE QM from "Active" to "State Option".
- Change the status for item K5i (Nutritional Approaches: None of Above) on REC_TYPE QM from "Active" to "State Option".
The rationale for these changes follows.
Combining a Quarterly Assessment with a Medicare PPS Assessment
To reduce provider assessment burden on Medicare PPS assessments, the new MDS Medicare PPS Assessment Form (MPAF) is being implemented in July, 2002. Starting in July 2002, facilities can choose to complete and submit the shorter MPAF form rather than a full MDS assessment for Medicare PPS assessments.
In some cases, a standard OBRA quarterly assessment and a Medicare PPS assessment will be due at the same time. When that occurs, the facility is allowed to perform a single assessment, combining the quarterly and Medicare PPS assessment. The combined assessment must indicate a quarterly assessment as primary reason for assessment (AA8a = 05 or 10) and a Medicare PPS assessment as the supplemental reason for assessment (AA8b = 1, 2, 3, 4, 5, 7, or 8) and have REC_TYPE of QM. The combined assessment must include all items on the MPAF form plus any additional items required by the state on quarterly assessments. In states that use a minimum 2-page quarterly, a RUG-III quarterly, or a RUG-III 1997 Update quarterly, the MPAF form will satisfy both the quarterly and Medicare PPS requirements.
Errors in Version 1.20 of the MDS Data Specifications have caused some confusion concerning a combined quarterly and Medicare PPS assessment. The specifications indicate that 5 items (G6f, H2e, H3j, J1p, and K5i) are active (required) on a combined quarterly and Medicare PPS assessment (REC_TYPE QM). However, these 5 items are not included on the MPAF form. The 5 items are "none of the above" items on incomplete checklists and the items were intentionally omitted from the MPAF form to reduce the size of the form. These items are intended to be active by state option rather than always active on a QM record. Unless the state specifically requires these items on a combined quarterly and Medicare PPS assessment (e.g., the state requires a full assessment as a quarterly), these 5 items will be inactive.
Correction 2 to Data Specifications Version 1.20 (05/21/2002)
One correction is required in the detailed Data Record Layout document (d_dt120.pdf). On page 7, in the "Consistency Required" for the REC_TYPE field the requirements for consistency between REC_TYPE AM and the reasons for assessment (AA8a/AA8b) is incorrect.
STATEMENT IN ERROR: *(2) If AA8a = 01 and AA8b = 1,2,3,4,5,7 or 8, then REC_TYPE must = AM.
CORRECT STATEMENT: *(2) If AA8a = 01 and AA8b = 1,5,7 or 8, then REC_TYPE must = AM.
