Interactive Atlas of Chronic Conditions
The interactive atlas of chronic conditions is a useful tool using GIS to examine geographic variability in chronic conditions. This new mapping tool shows the prevalence of 16 chronic conditions among Medicare beneficiaries at the state and county levels. The conditions Autism Spectrum Disorders, Hepatitis (Chronic Viral B & C), and HIV/AIDS are not presented in the map because more than one-third of the counties have suppressed information. The atlas presents data for 2015.

The map will automatically adjust between state and county levels as users zoom in or out. Zooming is available by clicking on the zoom buttons in the top left corner of the map, or by using the mouse wheel or keyboard "+" or "-" keys. Users can navigate the map by dragging with the mouse or by using the keyboard arrow keys. Also, the number of tabs displayed across the top that indicate the chronic conditions will change depending upon your screen size. If all 16 tabs are not displayed, the last tab will be blank and when selected will display a drop down showing the remaining conditions.
On October 1, 2015 the conversion from the 9th version of the International Classification of Diseases (ICD-9-CM) to version 10 (ICD-10-CM) occurred. Regardless of when a claim was submitted for payment, services that occurred prior to October 1, 2015, use ICD-9 codes. Chronic conditions identified in 2015 are based upon ICD-9 codes for the first ¾ of the year (January-September) and ICD-10 codes for the last quarter of the year (October-December).
