Medicare Billing: CMS-1450 & 837I

Claim Coding Information

Providers use a standardized coding system called Level II of the HCPCS mainly to identify products, supplies, and services not included in the CPT-4 codes, like ambulance services and DMEPOS when used outside a physician’s office.

Because Medicare and other insurers cover a variety of services, supplies, and equipment that CPT-4 codes don’t identify, the American Medical Association (AMA) and CMS established Level II HCPCS codes for submitting claims for these items. Find these codes in the HCPCS codebook or on the Alpha-Numeric HCPCS webpage.
