Medicare coverage of ABPM is essential for the diagnosis of hypertension. ABPM is required for the diagnosis of hypertension in all adults by the 2017 American College of Cardiology/American Heart Association Clinical Practice Guideline for High Blood Pressure in Adults. The scientific evidence supports the use of ABPM as the best method of excluding white coat hypertension, which may not require drug treatment, and to detect masked hypertension, which requires drug treatment. In my hypertension practice, the lack of reimbursement for ABPM has been a substantial deterrent in the diagnosis and treatment of hypertension. This is also true for almost all clinical practitioners. I strongly recommend that Medicare reimburse for ABPM.
