The comment period for the Fluid Jet System in the Treatment of Benign Prostatic Hypertrophy (BPH) DL38549 Local Coverage Determination (LCD) began on 6/1/2020 and ended on 7/7/2020. The notice period for L38549 begins on 11/12/20 and will become effective on 12/27/20. The title of the LCD was revised from Fluid Jet System in the Treatment of Benign Prostatic Hypertrophy (BPH) to Transurethral Waterjet Ablation of the Prostate.

Response To Comments


Comments were received regarding amendment to size of prostate from 30-80 mL to 30-150 mL.

Palmetto GBA agrees to amend prostate volume to 30-150 mL after careful review of very recently published literature.


Comments were received regarding removal of PVR > 300 mL limitation.

Palmetto GBA agrees to remove limitation #8 post void residual urine volume > 300 mL.


A comment was submitted indicating that the American Urological Association (AUA) includes aquablation in their 2019 BPH surgical guidelines. The commenter noted that a multicenter international water study spanning three years demonstrated that aquablation equaled TURP in efficacy. The study indicated a notable improvement for aquablation over TURP in significantly lowering impact in sexual function and medication restarts at three years (aquablation is 9% compared to 14% for TURP). The commenter hopes that coverage will be provided for Medicare patients.

We appreciate your comments, it is the intention to provide limited coverage for transurethral waterjet ablation of the prostate as outlined in this LCD.
