Webinar: Evidence-Based Interventions to Reduce Avoidable Hospitalizations
The Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents is a new initiative designed to improve care for people living in nursing facilities who are enrolled in Medicare and Medicaid.
Through this initiative, CMS will partner with independent organizations to improve care for long-stay nursing facility residents. These organizations will collaborate with nursing facilities and States to provide coordinated, person-centered care with the goal of reducing avoidable hospital stays.
In the second public webinar on this initiative, CMS staff will be joined by independent faculty who will discuss evidence-based interventions that have successfully reduced avoidable hospitalizations among nursing facility residents.
This session will include examples and discussions on the components of successful interventions, resources used to implement models, and potential barriers that may exist.
Note: CMS will not be discussing or answering questions regarding the technical requirements of the Initiative (e.g., eligibility, how to apply, etc.), and CMS is not endorsing any one or set of models. Questions of this nature may be submitted to NFInitiative2012@cms.hhs.gov. This session is being hosted to provide education on successful interventions to interested parties and the general learning community.
