CMS is transitioning to a process where LDS Data Use Agreement (DUA) requesters will submit and manage their DUAs through the DUA tracking system, EPPE.

Registration Instructions
LDS requesters are required to complete two trainings before accessing EPPE for the first time. Training materials are available as either a YouTube video or PDF slide deck. Users will need to attest to completing the training when registering for EPPE roles.

Important Information
- DUA Policies: Requesters will no longer be required to complete a paper version of LDS DUA Form 0235L (PDF) when submitting a DUA through EPPE. However, requesters will attest to and be expected to adhere to all policies listed on the form.
- DUA Custodian: Custodians are required to complete and sign the DUA Addendum (PDF) form as part of the EPPE process. While the requester adds individuals to a DUA directly in EPPE, CMS still needs a signed copy of the DUA Addendum form for legal purposes.
