Description
The purpose of this project is to design and conduct an evaluation of the Medicare Medical Home Demonstration. The 3-year project is authorized by Section 204 of the Tax Relief and Health Care Act of 2006 and amended by Section 133 of the Medicare Improvements for Patients and Providers Act of 2008. The demonstration will "provide targeted, accessible, continuous, and coordinated family-centered care to high needs populations." Under this demonstration, personal physicians will receive a monthly management fee payment for each Medicare beneficiary enrolled in the medical home demonstration. The demonstration will be conducted in a mix of large and small practices in up to 8 states. The evaluation will identify key features of practices providing medical home services to Medicare beneficiaries. The findings from the analyses will be used to prepare the annual Reports to Congress as directed by the legislation.
