Durable Medical Equipment Electronic Order Project
The Centers for Medicare & Medicaid Services (CMS) has initiated the Durable Medical Equipment Electronic Order (DME eRx) project to support the CMS Promoting Interoperability initiatives. This project will define exchange methods, use of other interoperability standards and specific uses of Fast Healthcare Interoperability Resources (FHIR) to effectively exchange orders for Durable Medical Equipment (DME) and associated supplies, medications, and supporting clinical information (to qualify the order or establish medical necessity) between the ordering provider and the intended supplier.
DME eRx Project Scope
The DME eRx workflow is based on the Health Level Seven (HL7) FHIR DME Order Implementation Guide (IG) and the DME Order Substitutable Medical Applications, Reusable Technology (SMART) on FHIR application. The DME Order SMART on FHIR application is a prototype of the HL7 FHIR DME Order IG. This SMART on FHIR application can also interact with other SMART on FHIR applications for CMS Documentation Requirement Lookup Service (DRLS). The DRLS is used when determining Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS) coverage requirements based on FHIR-based transactions. DRLS lists the items/services for which prior authorization is required and the documentation requirements for Oxygen and Continuous Positive Airway Pressure (CPAP) devices.
DME eRx Pilots and Workgroup
The EMDI Program supports and manages pilots that test the prototype of the DME order SMART on FHIR application. The EMDI team manages and monitors the DME eRx pilots through the DME eRx Workgroup.
The DME eRx Workgroup supports the pilots for the DME eRx workflow, which is based on the HL7 FHIR DME Order IG and DME Order SMART on FHIR application. This workgroup discusses pilot-related topics with the expectation of identifying how to successfully implement the SMART on FHIR application to achieve provider-to-provider interoperability. This workgroup is open to healthcare organizations interested in implementing the SMART on FHIR application and DME eRx Workflow.
