CMS oversees laboratory quality and performance through a variety of registries and data to ensure adherence to CLIA regulations. Here you can access information to help you understand the laboratory demographics across the United States, how laboratories perform, and regulatory action taken under CLIA.
Search for a CLIA Certificate and Laboratory Demographics
Visit Quality, Certification and Oversight Reports (QCOR) to access and reprint your laboratory’s CLIA certificate and find demographic information about laboratories certified under CLIA that perform laboratory testing. You can search by CLIA number, facility name and address where laboratory testing is performed, the type of CLIA certificate, and the date the certificate expires.

Analyze Laboratory Performance
Review the most current top ten laboratory deficiencies (PDF) found by surveyors during laboratory inspections. These common deficiencies show how well a laboratory performs and highlight opportunities for improvement to help laboratories strengthen safety practices to meet quality standards.
Access the Laboratory Registry
The Laboratory Registry, published annually by CMS, provides information that health care providers and the general public can use to evaluate laboratory performance. CLIA regulation 42 CFR 493.1850 requires this listing include:
- Laboratories convicted of fraud, abuse, false billing, or kickbacks under Federal or State laws.
- Laboratories with suspended, limited, or revoked CLIA certificates, and the reasons for the adverse actions.
- Individuals convicted of CLIA violations, circumstances of each case, and the penalties imposed.
- Laboratories with imposed alternative sanctions that include:
- effective date
- reason
- any corrective action taken by the laboratory
- compliance status, including verified compliance date
- Laboratories whose accreditation has been withdrawn or revoked and the reason(s)
- All appeals and hearing decisions
- Laboratories involved in CMS enforcement action under Section 493.1846 and the reasons for those actions
- Laboratories excluded from Medicare or Medicaid participation and the reason(s)
- Civil settlements reached with clinical laboratories
- Corrections from prior registries
- Additional information from CLIA-exempt states
- A final section with other information that may be useful to evaluate laboratory performance
CMS compiles the Laboratory Registry based on information from the previous calendar year. Refer to specific Laboratory Registry information for the past five years:

Review CLIA Hearing Decisions
Access a list of CLIA-related hearing decisions (PDF)and review guidance on each case, organized by hearing date. These decisions provide insight into how CMS enforces CLIA regulations.
Access Termination Notices
If CMS finds a laboratory to be noncompliant with CLIA requirements, CMS can suspend, revoke, or limit its CLIA certificate. In these situations, the laboratory can no longer perform testing or receive payment by Medicare or Medicaid for laboratory services. CMS informs the public about terminations across all provider types before they take effect.
