Make Sure Your Enrollment Application is Complete & Updated Fact Sheet

** Program Sponsors – We strongly encourage you to forward this message to your office managers, billing/coding and administrative staff. **

Don’t Forget to Submit Required Documentation with Your Medicare Enrollment Applications

As you submit your Opioid Treatment Programs’ (OTP) enrollment applications, make sure you include all required documentation. If you submit an application without all documentation, it will delay processing and your ability to bill Medicare.

Review the OTP Medicare Enrollment Fact Sheet (PDF) for a list of all required documents, and make sure your application includes:

  • Detailed organizational chart (like the one used for SAMHSA certification) showing the roles of your managing employees
  • Names, contact information, and Tax Identification Numbers (TINs) and/or Social Security Numbers (SSNs) for all individuals or organizations with managing control and/or ownership interest in the OTP – including your Medical Director and Program Sponsor
  • Signed provider agreement (PDF)

Updated Billing & Payment Fact Sheet

We’ve updated the Billing and Payment fact sheet (PDF) to include information on the threshold to bill for a full episode, as well as billing and payment for patients dually eligible for Medicare and Medicaid.
