The Requirements and Best Practices webpage for the Hospice Quality Reporting Program (HQRP) provides updates about reporting requirements and best practice methods to help hospices succeed with the HQRP.

This webpage provides resources for the HQRP overall, which currently includes the Hospice Item Set (HIS), the Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Hospice survey, and administrative data (Medicare claims). Specific resources available on this page include a Provider Toolkit section with “Getting Started” documents and tip sheets for all current and new providers.

In addition, the Downloads section contains quarterly update documents, which include frequently asked questions from the Hospice Quality Help Desk. Other reference sheets can also be found here.

Informational messages related to the HQRP are sent to hospices on a quarterly basis by Swingtech. Their latest outreach communication can be found below. Providers who would like to receive Swingtech’s quarterly emails can add or update the email addresses by sending an email to QRPHelp@swingtech.com. Make sure to include your facility name and Centers for Medicare & Medicaid Services (CMS_ Certification Number (CCN) along with any requested updates.
