ASPIRE (Accelerating State Pediatric Innovation Readiness and Effectiveness) Model

The Accelerating State Pediatric Innovation Readiness and Effectiveness (ASPIRE) Model will support whole-person care delivery for children up to age 21 who are enrolled in Medicaid and the Children’s Health Insurance Program (CHIP) who either have or are at risk of developing complex medical and/or behavioral needs, securing a healthier future for some of America’s most vulnerable youth.

ASPIRE is a state-based, voluntary model that will run for 10 years and up to five states will be eligible to participate. CMS will release a Notice of Funding Opportunity (NOFO) in 2026.

  • Problem: Children and youth with complex medical and/or behavioral health needs often face obstacles accessing necessary care, which can have lifelong implications like poor health outcomes or requiring care in more intensive settings outside of the community, and approximately half of this vulnerable population is covered by Medicaid and/or CHIP.
  • Solution: ASPIRE will position Medicaid providers to assume accountability for quality and cost of care for these populations, providing children, youth, and their families with care coordination and other supports across the continuum of physical and behavioral health, while incorporating quality measures focused on outcomes. 
  • Outcomes: Identifying and addressing the needs of children and youth early in life can help promote timely diagnosis that enables early intervention, to help them thrive in the least restrictive settings, ensure they will receive the right care at the right time, and improve quality of life for children, youth, and their families.
  • Strategy: ASPIRE reflects the Innovation Center’s commitment to helping children and youth live healthier lives by using evidence-based approaches to prevention and empowering families through greater access to comprehensive and coordinated services aligned with the goals of their child.
