
White Earth Long Term Care Consultation Services

White Earth Band of the Ojibwe formed the Long-Term Care Consultation (LTCC) program in 2005.
At that time, White Earth began contracting with the state of Minnesota to provide case management and eligibility assessments for home- and community-based services (HCBS).
By administering several Medicaid waivers, White Earth LTCC now provides an array of HCBS.

Eligibility
White Earth LTCC serves an average of 300 clients each month. Services are available to non-Native beneficiaries, but limited to people who live on the White Earth Reservation.
Funding
White Earth LTCC provides much of its services through several 1915(c) HCBS waivers under Medicaid. The tribe, Medicare, and the Department of Veterans Affairs provide the rest.
