The CMS Office of Minority Health held the "Solutions to Reduce Disparities: CMS Equity Plan for Medicare One Year Later" webinar to highlight how health equity was embedded across policies and programs during the first year of implementing the CMS Equity Plan for Improving Quality in Medicare. CMS Rural Health Solutions Summit

The CMS Rural Health Solutions Summit was held on October 19, 2016 at CMS Headquarters in Baltimore, MD. CMS Tribal Medicare Chronic Care Management Webinar

Many IHS, Tribal and Urban Indian programs provide chronic care management services and don't realize their program can receive Medicare Part B reimbursement. This webinar provided an overview of the CMS Chronic Care Management program and discussed the updated program with additional billing codes and requirements.

New Proposals for RHCs and FQHCs on Care Management Services and ACO Assignments - Listening Session

During this call, CMS experts reviewed proposals for Rural Health Clinics (RHCs) and Federally Qualified Health Centers (FQHCs) in the Physician Fee Schedule proposed rule on requirements and payment for Care Management Services, which includes Chronic Care Management (CCM), General Behavioral Health Integration (BHI), and Psychiatric Collaborative Care Model (CoCM) services. Learn more about the CMS CCM Campaign and the proposed new process for using RHC and FQHC claims to assign beneficiaries to Accountable Care Organizations (ACOs) participating in the Medicare Shared Savings Program.

Medication Safety LAN: What Can Chronic Care Management Do For You?

What does Chronic Care Management (CCM) services mean to you as a patient or caregiver? How do other members of the care team, such as pharmacists, assist in provision of CCM services? During this Medication Safety Learning and Action Network (LAN) event, presenters explained the impact of CCM and provided new resources and strategies that care teams can utilize for reimbursement and sustainability of CCM services. This webinar was designed for beneficiaries, families, health care providers, practitioners, partners, and Quality Innovation Network-Quality Improvement Organizations.

Connected Care: What Nurses Should Know About Chronic Care Management

The CMS Office of Minority Health co-hosted a webinar with the National Association of Hispanic Nurses to share the latest updates from CMS experts on chronic care management services. The webinar featured new resources available through Connected Care, a program aiming to educate health care professionals and consumers about the benefits of providing chronic care management to Medicare patients with multiple chronic conditions.

Connecting to Chronic Care Management Services

The CMS OMH and HRSA FORHP held a webinar to introduce the Connected Care initiative designed to educate health care professionals and consumers of the benefits of chronic care management (CCM) services. Professional, health system, patient, and caregiver organizations were encouraged to join this effort and participate in the webinar.

Understanding and Promoting the Value of Chronic Care
Management Services

CMS held a National Provider Call focused on CCM services. During this call, CMS experts discussed the benefits of providing CCM services and changes for CCM in the Physician Fee Schedule Final Rule. The Connected Care public education campaign was also discussed.

Delivering Coordinated Care through Chronic Care
Management Services

In November 2016, the CMS Office of Minority Health hosted a webinar entitled “Delivering Coordinated Care through Chronic Care Management Services” to share information and respond to questions on CCM services and the Medicare Physician Fee Schedule Final Rule changes that enable practitioners to receive payment for non-face-to-face care coordination services provided to Medicare and dual eligible beneficiaries with multiple chronic conditions.
