2026 Physician Fee Schedule Final Rule resources

• 2026 Total Per Capita Cost (TPCC) measure specifications (ZIP) that includes the cost measure codes list and an updated measure information form (MIF) for the modified TPCC measure that has been finalized in the Calendar Year (CY) 2026 Physician Fee Schedule final rule.

Care episode & patient condition codes

• 2026 Operational List of Care Episode & Patient Condition Codes (XLSX) includes revisions as finalized in the CY 2026 Physician Fee Schedule final rule to reflect service and diagnosis code updates identified through the annual maintenance of cost measures.

2025 MIPS cost performance category resources
