This page presents Public Use Files (PUF) and other publicly-available data related to the CMS End-Stage Renal Disease (ESRD) Program. The data files found here contain data from one or more of the four IT systems which comprise the ESRD Quality Reporting System (EQRS): REMIS, CROWNWeb, ESRD QIP, and EQRS. Together these systems serve as the congressionally-mandated System of Record (SOR) for all ESRD Patients in the U.S. both Medicare-entitled and non-Medicare. SOR #09-70-0520 is also known as the Program Management and Medical Information System (PMMIS). Comments on the data files presented on this page may be directed to EQRS_ISG_IPT_Team@cms.hhs.gov.
