Approximately one week after the Special ODF has passed, the announcement, transcript and MP3 audio file will be posted under the "Downloads" section in one Download link for 30 days.

**Updated: July 13, 2018 - Special Open Door Forum (SODF): The IMPACT Act and Standardized Patient Assessment Data Elements, Wednesday, July 25, 2018 from 2:00-3:00pm Eastern Time. This SODF will provide information and solicit feedback pertaining to the Standardized Patient Assessment Data Elements (SPADE) work under the Improving Medicare Post-Acute Care Transformation Act of 2014 (commonly referred to as the IMPACT Act). This SODF will provide an update on the recent SPADE activities including progress on national field test data collection, early feedback from providers participating in the beta data collection, and upcoming stakeholder engagement activities that will help stakeholders remain engaged and informed during the upcoming year. We invite questions, comments, and ideas from providers, patients, consumers, researchers, and advocates in advance or during the Forum. **Updated (Conference ID & Number): July 11, 2018 - Special Open Door Forum (SODF): Templates and Clinical Data Elements (CDEs), Draft Release 2, Thursday, July 12, 2018 from 2:00-3:00pm Eastern Time. To allow physicians, provider and supplier professional associations, and/or all other interested parties to provide feedback on Draft Release 2 of the Home Health Templates and CDEs that are posted on the CMS.gov website. CMS held a SODF for Home Health Templates and CDEs on March 1, 2018. The updates reflected in Draft Release 2 are a result of the comments and recommendations that CMS received following the previous SODF. The updated printable clinical templates and suggested CDEs describe the data elements that CMS believes would be useful in supporting the documentation requirements for coverage of Home Health Services as follows: 1. Plan of Care & Certification, and 2. Face-to-Face Encounter. The updated Draft Release 2 Home Health Templates and CDEs will be available on the CMS.gov website just prior to the SODF, and can be accessed through the link below: https://www.cms.gov/Research-Statistics-Data-and-Systems/Computer-Data-and-Systems/Electronic-Clinical-Templates/Other-Benefit-Templates/Other-Templates-home-health.html. Providers and IT vendors can choose whether or not to use the printable clinical templates and/or the suggested CDEs. The suggested CDEs aren't a form, but a tool that IT vendors can integrate into electronic health record (EHR) systems to create prompts to assist providers with their medical record documentation for Medicare purposes. The templates and CDEs are intended to be a helpful guide, but every section may not apply to all types of patients’ services. Providers can choose the information or CDEs that do apply. Once completed by the physician or other practitioner, the resulting documentation would become part of the medical record. To better assist providers in medical record documentation, for Medicare Fee for Service purposes, CMS has developed lists of clinical elements within a printable clinical template. This development will allow EHR vendors to create prompts to assist the practitioner when documenting. These Home Health Templates and CDEs are some of the first of many on the list of topics that CMS is currently working to address. The printable clinical templates and suggested CDEs listed below, which describe the data elements that CMS believes would be useful in supporting the documentation requirements for coverage of Vitamin and Metabolic Assays, are as follows: 1. Order, and 2. Progress Note.
