Medicare Ground Ambulance Data Collection System

Calendar Year (CY) 2026 Physician Fee Schedule (PFS) Final Rule
CMS issued the CY 2026 PFS final rule that announces the finalized policy changes for Medicare payments under the PFS and other Medicare Part B payment policy issues including ambulance services. Section 2203 of the Full-Year Continuing Appropriations and Extensions Act, 2025 (Pub. L. 119-4, March 15, 2025) extended three existing add-on payments to the ambulance base and mileage rates under the Ambulance Fee Schedule for ground ambulance transports through September 30, 2025. Accordingly, CMS finalized the proposals to revise our regulations at 42 CFR §414.610(c)(1)(ii) and 414.610(c)(5)(ii) in this final rule to align with existing law.

Fact Sheet: CY 2020 PFS Final Rule CY 2020 PFS Proposed Rule Section 50203 (b) of the Bipartisan Budget Act (BBA) of 2018 requires the Secretary to develop a data collection system to collect cost, revenue, utilization, and other information determined appropriate with respect to ground ambulance providers suppliers. CMS proposed the establishment of the GADCS to meet the statutory requirement:
- Proposed the data collection format and elements,
- Proposed a sampling methodology to identify ground ambulance organizations for reporting each year through 2024 and not less than every 3 years after 2024, and reporting timeframes.
- Proposed a 10% payment reduction in payments that would otherwise be made to a ground ambulance organization that is identified for reporting but fails to sufficiently submit data,
- Proposed a process under which a ground ambulance organization can request a hardship exemption or informal review that, if granted by CMS would allow it to avoid the payment reduction.
