Application Types & Timeline

Now available: Notice of Intent to Apply and application cycle dates for a January 1, 2021 start date.

ACOs may apply to the Shared Savings Program, for a Skilled Nursing Facility (SNF) 3-Day Rule Waiver, and/or to operate a Beneficiary Incentive Program (BIP). The application cycle follows the process outlined below. 

The sample applications for a January 1, 2020 start date provided below are for reference only; all ACOs must apply to the Shared Savings Program through the ACO Management System (ACO-MS).

Sample Applications for a January 1, 2020 Start Date*

*The sample applications for the January 1, 2020 application submission are for informational purposes only. They will be updated for the January 1, 2021 application submission. 

  • Initial Application (PDF): Available for ACOs not currently participating in the Shared Savings Program (initial and re-entering applicants). 

  • Renewal/Early Renewal Application (PDF): Available for ACOs applying to continue their participation in the Shared Savings Program without interruption, which includes ACOs that need to enter a new agreement period because their current ACO Participation Agreement is expiring, as well as ACOs that voluntarily terminate their current ACO Participation Agreement and are applying to immediately enter a new agreement period (early renewal). 

  • SNF 3-Day Rule Waiver Application (PDF): An additional application available for initial and renewal applicants applying to or ACOs participating in the Shared Savings Program under Levels C, D, or E of the BASIC track, or the ENHANCED track, as well as ACOs currently participating in the Shared Savings Program under Track 3 (herein referred to as the ENHANCED track) or the Track 1+ Model. 

  • BIP Application (PDF): An additional application available for initial and renewal applicants applying to or ACOs participating in the Shared Savings Program under Levels C, D, or E of the BASIC track, or the ENHANCED track, as well as ACOs currently participating in the Shared Savings Program under Track 2 or Track 3 (herein referred to as the ENHANCED track).
