Application Types & Timeline

ACOs may apply to the Shared Savings Program, to the Medicare ACO Track 1+ Model (Track 1+ Model), and/or for a Skilled Nursing Facility (SNF) 3-Day Rule Waiver. The annual application process begins in the spring and follows the process and timeline outlined below. Note that materials on this page are relevant for the 2018 application cycle. We will update this page in advance of the 2019 application cycle.

The sample documents provided below are for reference only; to apply to the Shared Savings Program, do so through the ACO Management System (ACO-MS).

Application Types

Initial Application: Available for ACOs not currently participating in the Shared Savings Program (initial applicants). Download Sample
Renewal Application: Available for currently participating ACOs with a 2015 start date that intend to renew their participation agreement with CMS (renewal applicants). Download Sample
Track 1+ Model Application: An additional application available for initial and renewal applicants applying to the Shared Savings Program under Track 1 and ACOs currently participating in the Shared Savings Program under Track 1. Applications will be accepted for 2018, 2019, and 2020 program start dates. Download Sample
SNF 3-Day Rule Waiver Application: An additional application available for initial and renewal applicants applying to the Shared Savings Program under Track 3, ACOs currently participating in the Shared Savings Program under Track 3, and ACOs applying to the Track 1+ Model. Download Sample
