How they met the pledge

FlyteHealth is a digital health platform that helps patients prevent and manage obesity and cardiokidneymetabolic (CKM) conditions. To support the CMS Health Tech Ecosystem, we implemented secure patient identity verification, enabling patients to seamlessly access their connected health information through a trusted identity provider. 

Patients can view medications, laboratory results, conditions, allergies, and other clinical information alongside patient-generated health data, including weight, blood pressure, nutrition, activity, and medication adherence. FlyteHealth transforms this information into personalized care plans, AI-powered guidance, and educational content while enabling patients to schedule care with our multidisciplinary clinical team. Patient-generated health data and care updates can be securely shared with healthcare providers, supporting coordinated care across the healthcare ecosystem.
