How they met the pledge

MyMedAccess was built to eliminate the clipboard. We saw firsthand in a primary care setting how patients repeated their health history at every visit, filled out the same forms, and manually carried records between providers. We built MyMedAccess to solve that problem at the source.

MyMedAccess connects to a patient's Medicare health plan and participating providers via FHIR-based APIs, automatically retrieving medications, lab results, diagnoses, and visit history directly to the patient's device. Patients verify their identity once using ID.me, the same service used by Medicare.gov, and their records are ready.

When a patient arrives at a new provider, they generate a secure QR code in seconds. The provider scans it and sees everything they need, no portal login, no fax, no phone call. Every share is patient-initiated, scoped to what the patient consents to, and expires after the visit.
Designed with a security-first approach using enterprise-grade data loss prevention technology and built with direct physician input from real clinical situations. Independently certified under the CARIN Code of Conduct by DirectTrust.
