How they met the pledge

WellConnector built a patient-controlled digital health profile that lets patients transmit their health history and insurance information to any point of care using FHIR-based Smart Health Links and QR codes, eliminating repetitive intake paperwork. At check-in, providers can accept structured patient data via QR code or Smart Health Link, and patients can retrieve a FHIR-formatted visit summary — notes, diagnoses, instructions — through the same method, presented in a user-friendly view within the app. Dependent management features help make caregiving more manageable.

The platform's FHIR-native data layer supports category-level sharing permissions, so patient consent preferences travel with each transaction and are honored across all parties involved, including treatment use cases. Identity verification is handled through a CMS-approved service (ID.me), with end-to-end encryption and zero-knowledge architecture ensuring patients remain the sole authoritative source of their own health data across every provider interaction.
