Welldoc’s digital health solution supports individuals managing diabetes, hypertension, obesity, and pre-diabetes while adhering to the CMS Medicare ACCESS pledge.
To fulfill CMS technical directives, our app uses IAL2 identity verification and connects with CMS Aligned Networks through TEFCA.
The solution allows patients to ingest disparate healthcare data and connect with over 400 devices and systems to provide personalized coaching, reminders, and risk-based alerts for their conditions within a single, FDA-cleared, DiME Seal approved, HIPAA compliant and cybersecurity certified app. AI-driven real-time and longitudinal coaching is delivered across six dimensions: Medications, Education, Diet, Activity, Labs, and Symptoms (MEDALS).
It also provides robust educational tools for nutrition and lifestyle management. It allows patients to filter data subsets or entire data sets, and share these data sets and progress reports with clinicians in their respective operational workflows without the need for portals.
