Webinar: Comprehensive ESRD Care Initiative - Building Effective Partnerships

Pioneer ACOs participated in a facilitated round table discussion regarding what considerations were made when identifying potential partners. Panelists shared thoughts on how to assess ideal characteristics of potential ESCO (ESRD seamless care organizations) partners, how they created a shared mission statement, what changes were made to their leadership structure, and share lessons learned related to their implementation plan. Audience members were encouraged to engage with the panelists through questions and answers.
