The Provider Engagement Opportunities webpage is where CMS posts opportunities for hospices to be involved in Hospice Quality Reporting Program (HQRP) measure and item development activities such as calls for Technical Expert Panel (TEP) participation, and calls for applications for pilot tests. This page also includes in the Downloads section, documents related to these provider engagement activities such as the TEP Reports and Measures Under Consideration (MUC).

Future Quality Measure Development Work

Background

As stated in the FY 2016 Hospice Wage Index and Payment Rate Update and Hospice Quality Reporting Requirements final rule (80 FR 47188), based on input from stakeholders, CMS identified several high priority areas for future measure development, including claims-based measures focused on care practice patterns. CMS continues to work with our measure development and maintenance contractor to identify measure concepts for future implementation in the HQRP.

In identifying priority areas for future measure enhancement and development, CMS takes into consideration input from all stakeholders including the Measures Application Partnership (MAP), the Medicare Payment Advisory Commission (MedPAC), Technical Expert Panels (TEP), and national priorities, such as those established by the National Priorities Partnership, the HHS Strategic Plan, the National Strategy for Quality Improvement in Healthcare, the CMS Quality Strategy and the general public, such as through rulemaking. In addition, CMS considers feedback and input from published research and reports.

For more information on measure concepts under consideration, CMS encourages the public to review the MUC list and the MAP report, which are both published annually. Quality Measures Currently Under Development

Transitions from Hospice Care, Followed by Death or Acute Care, Measure CMS is developing a claims-based Transitions from Hospice Care, Followed by Death or Acute Care, Measure (“Transitions from Hospice Measure”), as noted in the FY 2016 and FY 2018 Hospice Wage Index and Payment Rate Update and Hospice Quality Reporting Requirements final rules (80 FR 47188 and 82 FR 36657, respectively).The intent of the Transitions from Hospice Measure is to assess negative outcomes following hospice live discharge, including death or acute care use shortly after discharge, as these outcomes represent potentially burdensome transitions to patients and families. This measure is calculated using Medicare claims data so if implemented in the HQRP, there will be no additional data submission requirements. As with any new quality measure, the Transitions from Hospice Measure will be formally proposed through CMS fiscal year hospice rulemaking prior to its implementation.

CMS follows the CMS Measure Management System Blueprint process to develop quality measures. Through this process, we commit to engaging stakeholders throughout the development of the measure. In May, 2017, the CMS measure developer conducted TEP meetings to solicit stakeholders’ input on both the concept and the specifications of the measure (See “May 2017 TEP Claims Based Quality Measures Development for Hospice Quality Reporting Program” under the Downloads section). Additionally, a call for public comments on the measure was available from March 27, 2018 through April 25, 2018 to solicit comments for the MAP process. (Please see “HQRP Transitions QM Public Comment Summary Report” under the Downloads section for summary and CMS responses to the public comments). These activities are intended to provide opportunities for stakeholders to learn more, and to submit their measure comments. The Transitions from Hospice Measure will be included on the 2018-2019 MUC list that will be posted by December 1, 2018.

Stakeholders may comment on the posted MUC list, that will include the Transitions from Hospice Care Measure, prior to the NQF Post-Acute Care/Long-Term Care Workgroup (December, 2018) and NQF Coordinating Committee Workgroup (January, 2019). CMS reviews comments submitted through the MAP and considers them and the MAP recommendations before implementing any new quality measure. Your valuable comments are considered, and become part of the ongoing measure development. Hospice measures recommended by the MAP will then be proposed in the next CMS fiscal year Hospice Wage Index and Payment Rate Update and Hospice Quality Reporting Requirements rule allowing stakeholders with another opportunity to comment on the measure’s use in the HQRP.
