The purpose of this page is to provide access to materials from past HQRP trainings and educational events. Materials on this page are organized by event and materials from each event (e.g., PowerPoint slides, worksheets, Q&A documents) are posted in the .zip files in the “Downloads” section below. Providers can browse this training library for topics of interest and use the associated downloads to increase their knowledge on topics pertinent to the HQRP.

Older trainings are moved to the Hospice Quality Reporting Archives page. Trainings on the Archives page may be helpful historical references to providers, but have been archived because at least some portion of their content is out-of-date.

Hospice Quality Reporting Program Webinar: From Data to Measure

What you will learn:

In this webinar, you will learn how HIS data is used to calculate HIS quality measures (or QMs), including a discussion of how to use the quality measure specifications in the QM User’s Manual and how publicly reported scores are determined. The training also provides guidance on how providers should interpret their QM Reports (both the Hospice-level QM Report and the Patient stay-level QM Report) to understand their hospice’s quality performance. This webinar is intended for advanced HIS audiences. It is recommended that providers have a basic understanding of the HIS, a working knowledge of the HIS-based QMs, and some familiarity with the QM reports to maximize benefit from this webinar.

Navigating the HQRP Websites Modules Series

What you will learn:

This series of modules presents information to help hospice providers navigate the three websites related to the Hospice Quality Reporting Program (HQRP). This series consists of 5 on-demand video modules that providers can watch to learn more about each website, what information is contained on each, and how each website is organized. PowerPoint slides for each module are also available for download.

The modules are designed for providers to watch independently or as a group. Each module is described below:

  • Module 1: Introduction to the 3 HQRP Websites: this module provides an overview of the 3 websites pertinent to the Hospice Quality Reporting Program (HQRP), which are the CMS HQRP website, the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Survey website, and the QIES Technical Support Office (QTSO) website.
  • Module 2: Learning How to use the CMS HQRP Website: this module takes a deep dive into the organization and structure of the CMS HQRP website and will help you understand how content on the website is organized so you can gain confidence in finding information on the website, even as website content is updated over time.
  • Module 3: Learning how to use the CAHPS® Survey Website: this module shows the key parts of the CAHPS® Hospice Survey website that are pertinent to providers. This module will show you where to find key Hospice CAHPS® forms, CAHPS® podcasts, and FAQs.
  • Module 4: Learning how to use the QTSO Website: this module will show providers key portions of the QTSO website that are relevant to hospice, including learning where to find information and User Guides for the Hospice Abstraction Reporting Tool (HART), Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP) system, and the Certification And Survey Provider Enhanced Reports (CASPER) reporting application. This module will also show providers where on the QTSO website scheduled QIES ASAP downtimes are announced.
  • Module 5: An Overview of the HQRP Help Desks: this module will provide contact information and information on the scope of each Help Desk available to hospice providers.

May 2018 Hospice Quality Reporting Program (HQRP) Data Submission and Reporting Webinar

What you will learn:

This training presents information to help providers understand the reports that are available through the Hospice Quality Reporting Program, as well as to understand and access the information contained within these reports.  

Topics included:

  • How to navigate the CASPER (Certification And Survey Provider Enhanced Reports) system
  • An introduction to the Hospice Item Set (HIS) and CAHPS® Provider Preview Reports  
  • A brief review of information available in previously released reports.

HIS Coding Modules Series

What you will learn:

This series of modules presents information on updated HIS coding guidance for selected HIS items. This series consists of 8 on-demand video modules that providers can watch to learn more about how to code select HIS items in specific clinical situations. PowerPoint slides for each module are also available for download.

The modules are designed for providers to watch independently or as a group. Note that these modules are intended for users familiar with the HIS. If you are new to the HIS, we recommend you review the HIS Manual (available here) and HIS V2.00.0 training modules (available on this webpage) before viewing this series. Each module is described below:

  • Module 1: General Information: this series of modules covers select HIS items only; the content presented in these modules addresses frequently asked questions received on the Hospice Quality Help Desk. As such, these modules present refined guidance (refined instructions and detail for coding HIS items that addresses specific clinical scenarios or frequently encountered situations) for select HIS items and are intended to be a supplement to existing guidance in the HIS Manual. Consequently, this series of modules does not cover all HIS items. This module presents general information on the HIS, including HIS completion and submission deadlines, who can complete and submit HIS records, and the difference between HIS item completion and quality measure credit.
  • Module 2: Section A, I, and Z: this module presents updated coding guidance for sections A, I, and Z in the HIS, including guidance on the ZIP Code item (A0550), payor information item (A1400), principal diagnosis item (I0010), and the record completion (Z0400) and verification (Z0500) items.
  • Module 3: Section F, Preferences: this module presents information on the intent behind items in Section F, which include the patient preferences items (F2000-F2200) and the spiritual/existential concerns item (F3000).
  • Module 4: Section J, Pain: this module presents information on items in Section J, Pain, including describing how providers receive credit for the pain screening and comprehensive pain assessment quality measure. This item also presents information on warning edit -3077 which appears when the patient is on an opioid, but pain is marked as not an active problem.
  • Module 5: Section J, Respiratory Status: this module presents information on the shortness of breath items and the associated quality measures.
  • Module 6: Section N, Medications: this module presents information on the medications items, which include the PRN and scheduled opioid items, as well as the bowel regimen item. This module also describes the intent of the associated quality measure, and why bowel regimens initiated for non-opioid induced constipation are not included in the items/measure.
  • Module 7: Section O, Service Utilization: this module presents information on the visits and level of care items in Section O, including guidance on what counts as a visit and whose visits can be counted for these items. This module also describes the Hospice Visits when Death is Imminent Measure Pair.
  • Module 8: Chapter 3 Submission and Correction of HIS Records: This module presents information on the process for correcting errors in HIS records.

Reporting Hospice Quality Data: Tips for Compliance Call

What you will learn:

This training presents information to help the hospice providers be compliant and successfully submit Hospice Item Set (HIS) data and the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Survey. Topics included:

  • HIS and CAHPS® submission requirements
  • Reasons for noncompliance and how to address them
  • Timelines for data submission and compliance determinations
  • Resources for success, including how to access important websites and helpdesks

Hospice Item Set (HIS) V2.00.0 Training

What you will learn:

This train-the-trainer presentation focuses on updates made to the HIS V2.00.0, which become effective on April 1, 2017.Topics include the two new HIS-based quality measures that will be incorporated into the HQRP April 2017 (Hospice Visits when Death is Imminent and Hospice Composite Measure), as well as new items added to the HIS V2.00.0 in: Section A (Items A0550, A1400); Section J (Item J0905), and Section O (Items O5000 – O5030). This training also reviewed existing items on from the HIS V1.00.0. The training also included a focus on the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey and offer important insights into frequently asked questions regarding the HQRP. This training also included 2 case studies where providers can test their knowledge of content included in the training.

Final Validation Reports Webinar

What you will learn:

This webinar details HQRP submission requirements including the HIS related and Hospice CAHPS requirements, provides useful insights into how to download and use the Hospice Final Validation Reports and other Certification And Survey Provider Enhanced Reports (CASPER) reports available to providers, and assists providers in understanding how to interpret error messages and correct errors. HIS Technical Training: Submitting HIS data and navigating QIES ASAP and CASPER

What you will learn:

Four training modules to assist providers successfully submit their Hospice Item Set admission and discharge records to CMS. Each module provides helpful tips including submission instructions, top ten errors, how to access and review the hospice final validation reports, available CASPER reports and suggested uses, as well as where to locate useful technical documentation and QTSO help desk contact information.

  • HIS Data Submission (Module 1 of 4) – Tips for successful HIS submission - this module outlines tips for successful HIS data submission, including CMSNet User ID and QIES User ID registration and tips for submitting HIS data.
  • HIS Final Validation Report (FVR) and Top Ten Submission Errors (Module 2 of 4) - this module outlines important information about accessing and reviewing the hospice final validation report and provides a summary of the 10 most frequent errors for quarters 1 and 2 of 2015.

Hospice Quality Reporting Program Webinar: Preview of the New Educational Series and Updates on Hospice CAHPS®

What you will learn:

In this webinar, CMS introduces and presents previews of the two, recently-released series of educational modules (“Navigating the HQRP Websites” and “HIS Coding” Modules Series). The intent of this webinar was to introduce hospice providers to the two aforementioned series of educational modules, including information on where to find the modules, who should watch the modules, and what information they cover. In this webinar, subject matter experts also provide updates on Hospice CAHPS®, and a question and answer session is held following these presentations.
