The Road to Equity: Examining Structural Racism in Health Care

During National Minority Health Month, the CMS Office of Minority Health hosted a two-day virtual forum to highlight CMS’s renewed focus on health equity, address structural racism in health care, and establish federal agencies’ roles as equity partners. Guest speakers from various federal and partner agencies shared their insight and experience during the forum, focusing on the impact of COVID-19 on health disparities as well as solutions to promote equity.

CMS Tribal Medicare Chronic Care Management Webinar

Many IHS, Tribal and Urban Indian programs provide chronic care management services and don't realize their program can receive Medicare Part B reimbursement. This webinar provided an overview of the CMS Chronic Care Management program and discussed the updated program with additional billing codes and requirements.

Medication Safety LAN: What Can Chronic Care Management Do For You?

What does Chronic Care Management (CCM) services mean to you as a patient or caregiver? How do other members of the care team, such as pharmacists, assist in provision of CCM services? During this Medication Safety Learning and Action Network (LAN) event, presenters explained the impact of CCM and provided new resources and strategies that care teams can utilize for reimbursement and sustainability of CCM services. This webinar was designed for beneficiaries, families, health care providers, practitioners, partners, and Quality Innovation Network-Quality Improvement Organizations.

Connected Care: What Nurses Should Know About Chronic Care Management

The CMS Office of Minority Health co-hosted a webinar with the National Association of Hispanic Nurses to share the latest updates from CMS experts on chronic care management services. The webinar featured new resources available through Connected Care, a program aiming to educate health care professionals and consumers about the benefits of providing chronic care management to Medicare patients with multiple chronic conditions.

Connecting to Chronic Care Management Services

The CMS OMH and HRSA FORHP held a webinar to introduce the Connected Care initiative designed to educate health care professionals and consumers of the benefits of chronic care management (CCM) services. Professional, health system, patient, and caregiver organizations were encouraged to join this effort and participate in the webinar.

Delivering Coordinated Care through Chronic Care
Management Services

In November 2016, the CMS Office of Minority Health hosted a webinar entitled “Delivering Coordinated Care through Chronic Care Management Services” to share information and respond to questions on CCM services and the Medicare Physician Fee Schedule Final Rule changes that enable practitioners to receive payment for non-face-to-face care coordination services provided to Medicare and dual eligible beneficiaries with multiple chronic conditions.
